Prof. V. Moutaftchiev

A study on the diastema between permanent central incisors - etiology and changes in mixed dentition

This subject of the study was to investigate the diastema between the maxillary central incisors, its frequency, etiology and changes in mixed and permanent dentition.

It is determined that the diastema with a size of 1 mm and more than 1 mm is often found by early mixed dentition /29,45 %/ and it can be considered as a temporary characteristic because it decreases in permanent dentition significantly /7,2 %/ due to self-regulation. The basic part of the diastemas which were found in mixed dentition /72,22 % / are with a small size /1 - 1,9 mm /. In the maxillary diastemas a basic part of the central incisors are with divergent axes /75,86 % / which turn into parallel and divergent after self-regulation of the diastema. The predominant small maxillary diastema and the divergent axes of the central incisors in early mixed dentition allow the use of appliances with tip the teeth.

In classification of the etiology of the diastema it is determined the role of tooth-teeth and tooth-maxillary discrepancies in the frontal region according to Tonn and Gerlach. Concerning the disputable etiologic factor - frenum of upper lip, it is determined that in early mixed dentition its attachment below or at the line of the services of the teeth /the central incisors/ in early mixed dentition can be considered as a temporary effect and this is not an obstacle for a self-regulation of the diastema.

The tests for whitening and mobility of the interdental gingiva and the incisor papilla should not be serve as criteria for abnormality of the frenum of upper lip-mixed dentition.
